THE CEDARSTEP GUIDE

Coming home from hospital: a checklist for families

Questions to ask the discharge team about equipment, support, medicines and who to contact once home.

Before buying equipment, ask the discharge team what has already been assessed and arranged. This checklist helps you collect answers; it does not decide whether someone is ready to leave hospital.

Before leaving the ward

  • Ask what support will be in place at home, when it begins and who provides it.
  • Check how the journey home and entry to the property will work.
  • Ask whether any equipment is being supplied and who will show the person how to use it.
  • Check the medicines information, follow-up arrangements and contact for questions.
  • If the person wants family or carers involved, make sure the team knows.

The NHS discharge guide explains the information and planning involved. Ask staff to clarify anything you do not understand before leaving.

Picture the first evening

Discuss the route from the front door to the chair, toilet and bed. Tell the team about stairs or access problems rather than assuming someone will manage.

Check that food, drinks, keys and a way to call for help will be available. Keep the written care information somewhere the person can reach.

If the arrangements don't match the need

Raise concerns with the discharge team. Ask who to contact if planned support does not arrive or the situation changes. The NHS planning page gives further routes for concerns.

These NHS pages describe England's services. Ask the local hospital team about arrangements in Wales, Scotland and Northern Ireland.
